Medical Forms

Each boy needs to have his parent/guardian fill out this medical form and turn it in to their den leaders at the first den meeting. Yes -- this is new for this year. The medical form will provide us a quick reference if there are any accidents during camping trips, hikes, etc.

Den leaders and assistant den leaders must also fill out the form above if they are under 40. For den leaders and assistant den leaders who are 40 or over, this form need to be filled out and filed.
